Journal Club : Dobutamine-Noradrenaline vrs Adrenaline in cardiogenic shock

Researchers have just released findings of a prospective randomised trial comparing adrenaline with dobutamine and noradrenaline for the treatment of "dopamine resistant" cardiogenic shock. The paper found that there was no difference in attainment of haemodynamic endpoints, but there was an increase in the likelihood of arrhythmias, transient lactic acidosis and splanchnic perfusion as measured by gastric tonometry. This is a small study with surrogate endpoints, but hints again that there are limited differences between the available inotropic agents.
